Salem Schools, Health Dept. plan H1N1 shots
Salem City Schools, in cooperation with the Salem City Health Department, hope to offer H1N1 vaccinations shortly.
Parents must follow several important steps in order for their children to receive the H1N1 vaccination. The H1N1 vaccination is strictly voluntary and free of charge. The school vaccination is available only to Salem resident pre-school and school-age children.
You must register online at https://h1n1vaccine.odh.ohio.gov/. You will need an email account to register and you must register each child you wish to have vaccinated.
Elementary students
Print out the form, sign it,and bring it with you to the vaccination site. Vaccinations will be given outside the regular school day at Salem High School. The date will be announced via the school phone system and in the Salem News as soon as it is known.
Junior/Senior High students
Print out the form, sign it, and sent it with your student to school where they will turn it at the main office.The high school and junior high vaccinations will occur during the school day. The date will be announced via the school phone system and in the Salem News as soon as it is known.
No student will be vaccinated without the proper signed paperwork.
If you do not have an email account or computer access, stop by Salem City School's administrative offices or the Salem City Health Dept. office for help in getting registered.
There will be limited resources to help you register the day of the vaccination, so you are highly encouraged to use the pre-registration process ahead of time as there is no guarantee that the state database system will be operating the day of the vaccination.
